Use this form to report concerns about ethics, conduct, safety, security, fraud, harassment, discrimination, or any other issue you don't feel comfortable raising through normal channels.

What we do not collect: your name, email, IP address, browser fingerprint, or any session identifier. This page sends only the text you type below to a private Slack channel reviewed by leadership. You may submit as often as you like.
Avoid including your name unless you want to identify yourself. Leadership cannot follow up with you directly unless you provide a contact method below.
